Nero was no stranger to counsellors, but his interactions had mostly been to ensure he did not have trauma, or to determine whether or not he was developing anti-social tendencies. These weren't specific to him, and were instead common practices considering that almost every citizen of the country had magic powers and military training.
A single citizen who had a psychotic break could do immense damage, and unfortunately that was not just a hypothetical assessment. Despite many mechanisms in place to treat soldiers and prevent such incidents, they still happened.
When Nero thought of Felix Blair, the KMA cadet at the top of the Crucible list, and his vacant eyes, the first thing that came to mind was that he had suffered some kind of mental trauma or injury. There was no way to know if he was right, since Nero did not have access to personnel files, but that didn't stop him from having guesses.
