This could only happen through one of two circumstances. One, if the cultivator suddenly awakened a kind of 'Sword Intent' for their preferred weapon. And second, if the cultivator became so entwined with their weapon that it would become entwined with their own cultivation!

She trusted her automobile so much, pushing it to its limits, treating it as her own true partner, that she started to awaken it as her actual weapon.
This happened frequently. The most recent example would be a famous Knight from the Order of the Heavenly Knights.
It was said that this specific Knight used only a wooden sword his entire life because of how he didn't have any money to buy a real sword.
He continued to use this wooden sword, despite all of its weaknesses, and continued to improve using it.
