At the medical center. 

Mark's brain nearly short-circuited under the little boy's soulful stare. 

Why? Infinite possibilities, that's why! 

It's not like he couldn't have kids. Safety measures? Pfft, useless against big data. There'd been cases before. And the kid's expressive face? Totally sucked him in. For a split second, he didn't doubt this could be real—until the boy's face flipped to a cheeky grin at Adam, and Mark realized it might just be a prank. 

"Whoa, okay…" Mark let out a long breath, shooting Adam a helpless look. 

"Happy Halloween! No ears, no treats—time to trick!" Adam laughed. "His inner ear's fine, just not connecting to the outer parts. I'll sort out the OR with the chief. Nurses and anesthesiologist? I've got you covered. George can be your first assist. Dr. Sloan, for the sake of all those future mini-Sloans who might pop up someday, how about a free surgery for this kid today?" 

"This little rascal!" Mark glanced at the boy, now back to his hopeful puppy eyes. With Adam laying it on this thick, how could he say no? "Alright, I'll do this pro bono surgery." 

"Thank you, Dr. Sloan! Thank you!" The kid lit up, gratitude spilling out. 

You eat what's near your mountain or fish what's in your river. His dad worked at the hospital restaurant, and with his knack for planning and smooth-talking charm, he'd landed this life-changing surgery. Without that? He'd be waiting 'til who-knows-when. 

He was still young, but once middle school hit, his deformed ears would make him a target. Seven years of that? He'd either turn into another Leonard or a full-on serial killer. Judging by his high EQ now, probably the latter. 

With a noble goal of breaking the messed-up logic of TV drama worlds, Adam—daily do-gooder—took the boy to knock on the surgical chief's door. A quick "Knock, knock, no ears, no treats!" swap from the classic Halloween line, and the chief chuckled, setting it up no questions asked. 

The kid read the room right. Without Adam vouching, the chief would've brushed it off with, "Sorry, charity slots are full—no ORs available." Hospitals need cash. Free surgeries are just PR stunts—minimal effort, minimal slots. But with Adam playing the saint, the whole place was suddenly dripping with love! ❤️ 

With the lead surgeon and OR locked in, Adam called George and the nurse-whisperer crew to handle the kid's prep. A small gig like this? No way Adam was scrubbing in himself. 

Off to the ER he went. 

Holiday tradition says the wildest stuff always hits the emergency room. 

"…This is the sickest pumpkin head I've ever carved! Next thing you know, blood's spraying everywhere—splattered all over my pumpkin face. Made it even more twisted!" 

Adam walked in to a chubby kid raving, all hyped up. "Sick" was his word, but his face screamed thrilled. 

"Stop talking—your face got more of it than you think," Lexie snapped, bandaging his severed index finger with a grimace. 

"Welcome to the med center's Halloween live show," Carter grinned, arms crossed. 

He was an intern too, but having done his med student stint here, he'd seen how bonkers and bizarre the hospital's Halloween could get. 

"Got nothing to do?" Adam strolled over, cutting off Carter's smug moment. "Where's the patient's finger?" 

"Dr. Duncan!" Carter dropped his pose quick, flashing an awkward smile. "It's with the patient—I'll clean it up now!" 

"It's stuck in the chainsaw," the chubby kid piped up, grinning like it was no big deal. 

He wasn't some tough guy or total psycho—morphine had kicked in, numbing the pain and juicing him with a giddy high. That's why he sounded unhinged. 

"Wait, where's my chainsaw? It was right here!" The kid turned, face dropping as he scanned around. "Hey! Who took my chainsaw?!" 

"Carter!" Adam barked. 

"On it!" Carter jolted, sprinting off to hunt it down. 

Buzzzz! 

Right then, the unmistakable whine of a chainsaw revved through the ER. 

"Damn it!" Adam's face tightened as he bolted toward the sound. 

Security checks made it tough to sneak guns or dangerous stuff into the hospital these days. But the ER? That's the weak spot. Ambulances roll in with critical patients—every second counts. You can't stop to scan them. They come in first, questions later. 

That buzzing? Definitely the kid's chainsaw from his pumpkin-carving spree. In a hospital, though, a chainsaw's as deadly as a gun. 

Adam charged ahead. 

Spotting Christina leading a crew toward the noise, he shouted, "Christina! Hold up!" 

She froze, pieced it together, and waved her interns back. Adam shot past them, danger senses quiet. With a quick glance at Christina, he went in solo. 

It was a double room. The bed by the door was empty. The inner bed's curtain was drawn, hiding whatever was happening as the chainsaw's growl echoed out. 

Christina and Carter peeked from the doorway, tracking Adam's steps, nerves on edge. 

No warning vibes, but Adam flipped on bullet-time anyway, fully prepped. He yanked the curtain back. 

A splash of red flew at him. 

He dodged sideways. 

"Holy shit!" 

"Oh my God!" 

Christina and the others at the door gasped, catching sight of the chaos as Adam sidestepped. Their jaws dropped. 

Inside, one guy—an African dude—sat on the bed, legs up, wielding the chainsaw with a wild grin. 

You'd think he was hacking up an enemy in some gory revenge flick. But nope—he was sawing his own right calf! 

He didn't flinch under their stares, just kept at it. Against a chainsaw, bone doesn't stand a chance for long. 

"Holy crap, this guy's hardcore! Total psycho!" The chubby kid, ignoring Lexie's protests, waddled over and gawked in awe. 

Christina's eyes went weird. 

Lexie couldn't even look. 

"Welcome to the med center's Halloween live show!" Adam smirked at a stunned Carter.
